Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 7/7/2025. The inspection started at 12:00pm and concluded at 1:00pm. There were four children present and one caregiver; the point total was sevenThe inspector reviewed four children?s records and three caregiver/household members records on site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Vendors shall maintain, and keep at the family day home, written or electronic information for each enrolled child, which shall be made available to the department's representative upon request. There was no information available for child #1, who was in attendance during the inspection.
Plan of Correction: The vendor will collect the required information from the parents of Child #4.
Standard 8VAC20-790-190-B
The vendor shall maintain information for each child enrolled, including the telephone number for each custodial parent's place of employment. There was no record of either parent's work phone number for Child #2.
Plan of Correction: The vendor will obtain the work phone number of the parent's of Child #2.
