8VAC20-790 Subsidy Program Vendor Requirements for Subsidy Providers
Inspector Notes
A Subsidy Health and Safety Inspection was conducted on 6/22/2023 from 11:00am through 12:30pm. There were 3 children directly supervised by the vendor. During the inspection the home, activities and interactions with children, emergency supplies, procedures and drills were inspected. During the inspection, the children were observed eating snacks and watching TV on their tablets on the vendors couch. The vendor is not currently trained to administer medication and the vendor stated that no medication was on-site. Three children's records, vendor information and household member information was reviewed.
Information gathered determined violations with applicable code or requirements and are listed on the Violation Notice.

Violations
8Evidence: The vendor stated that they do not document daily attendance.
Evidence: 3 out of 3 children's records did not contain proof of each child's identity. The vendor stated that the families had not provided proof of identity.
Evidence:
1. There was chips and Cheetos observed on the couch and floor where the children were sitting and walking.
2. Two kitchen cabinet doors were missing from the cabinet frames.
3. One cabinet door and one drawer were observed laying on the kitchen floor where children were walking.
Evidence: There were four cleaners and disinfectants observed in the cabinet under the vendors sink. The cabinet did not have doors and the cleaners were accessible to the children.
Evidence: The children's bathroom does not contain disposable paper towels. When asked how the children dry their hands, the vendor pointed to paper towels located in the kitchen.
Evidence: Child #1 was observed taking a nap on the vendors couch where two other children played.
Evidence: The vendor did not have tweezers available during today's inspection.
Evidence: The vendor did not document evacuation drills for April 2023 and May 2023. The vendor stated that evacuation drills were not practiced in April and May.
