During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated on May 23, 2025 and completed on May 28, 2025. The inspection started at 12:58pm and concluded at 1:56pm. There were four children present and one caregiver; the point total was eight. The inspector reviewed four children?s records and two caregiver/household member records on-site and electronically on May 28, 2025. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
6The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
The vendor did not have documentation of daily attendance.
The child's information shall include the required information.
Child #1, #2, #3 and #4's records did not include documentation of proof of the child's identity.
Child #3's record did not include documentation of the address and phone number for mother's place of employment. Child #4's record did not include documentation of the address and phone number for the mother and father's place of employment.
Child #3's record did not contain documentation of immunizations.
The caregiver's TB screening was dated October 5, 2022.
Each bathroom area provided for children shall be equipped with soap, toilet paper, and disposable towels or an air dryer within reach of the children.
The bathroom utilized by the children did not have any paper towels.
The caregiver placed Child #4 in an infant rocker seat and did not utilize the safety straps to secure the child. Child #4 leaned forward in the seat and fell face-first onto the floor, with the infant rocker chair landing on the child's back.
The first aid kit did not contain tweezers.
