Inspector Notes

An unannounced subsidy health and safety inspection (SHSI) was conducted at 12:13pm. Observed 3 children = 7 points with the vendor. Ratios and supervision were incompliance. The children were observed playing with eating a snack, playing with toys and puzzles and making ink stamp art. The vendor was actively playing with the children. The daycare areas of the home were found to be clean and sufficiently supplied with toys and equipment for the children. Areas of non-compliance with subsidy requirements and laws reviewed were found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Evidence:

4. Child #13-The emergency contact name, address and phone number lists the father which should be changed to someone other than adult.

Plan of Correction: The vendor said she would ask parents to complete the required information on their enrollment paperwork.

Based on records reviewed, the vendor/caregiver #1 did not complete the annual 3-hour subsidy training course titled Virginia Annual Health + Safety for Childcare Staff. Evidence: There was no documentation of the course being completed in the last year.
Plan of Correction: The vendor said she thought she had taken the course and will do it right away. The training certificate will be kept on-file for verification.
