Inspector Notes

A initial subsidy health and safety inspection (SHSI) was initiated and completed on March 26, 2026. The inspection started at 2:00 p,m. and concluded at 3:00 p.m. There was 1 child present and 1 caregiver; the point total was 2. The inspector reviewed 1 child?s record and 1 caregiver record on-site. This inspection included document review, tour of the home, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Vendors shall maintain the required information for each enrolled child.

The record for Child #1 (enrolled over 3 weeks) was missing the child's proof of identity and written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
