A subsidy health and safety inspection (SHSI) was initiated on 8/13/25 and completed on 8/16/25. The inspection started at 11:10 AM and concluded at 11:45 AM. There were 3 enrolled children present and 1 caregiver; the point total was5. The inspector reviewed 3children?s records and 3 caregiver/household members records on-site.
