Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, and a virtual tour of the home.
A subsidy health and safety inspection for a Vendor that has moved was initiated on September 2, 2021 and concluded on September 8, 2021. The Vendor stated there are no children enrolled or present. The Vendor is current in First Aid and CPR but is not MAT certified. The Vendor has an up to date Emergency Plan to address new location and has a current local agency permit.
