Inspector Notes

A subsidy health and safety inspection ( SHSI) was conducted on 9/15/2025 from 9:55 am- 10:35 am. There were 4 children with 1 caregiver. The inspector reviewed 4 child records and 2 household members records were reviewed on-site.

The information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

The vendor did not complete daily attendance on 9/10/2025, 9/11/2025, 9/12/2025 and 9/15/2025.
Plan of Correction: I have to fix that. Corrected during the inspection.

The child information was found to be incomplete.

Child A is missing proof of identity.

Child B is missing parent work address.
Plan of Correction: I will tell them today in the afternoon at pick up.
