Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced complaint inspection was conducted to investigate a complaint received by the department related to a forbidden action at the center. The inspection was initiated on 4/26/2021 and concluded on 5/11/2021 with an exit interview. The director was contacted by telephone, interviews were conducted and documentation was reviewed. The evidence gathered during the investigation did not support the allegation of non-compliance with standards or law and the complaint is determined to be not valid.
