This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

An unannounced monitoring inspection was conducted to investigate a complaint received by the department related to Staffing and Supervision and Nutrition and Food Services. The inspection was initiated on 9/28/2021 and concluded on 9/29/2021 with an exit interview. The director was contacted by telephone, interviews were conducted and documentation was reviewed. It was determined that the complaint was not valid due to insufficient corroborating evidence to substantiate the allegations.
