Inspector Notes

This inspection was conducted by licensing staff using a combination of an alternate remote protocol, including telephone contacts, documents review, and interviews and an in-person observation of the facility.

An unannounced complaint inspection was conducted to investigate an allegation received by the department related to a Forbidden Action. The inspection was initiated on 1/21/2022 and concluded on 3/2/2022 with an exit interview. Interviews and observations were conducted, and documentation was reviewed. It was determined that the complaint was not valid due to insufficient corroborating evidence to substantiate the allegations. There was, however, a violation issued as a result of the inspection.

Based on interviews, not all children under 10 years of age were always within actual sight and sound supervision of staff. Evidence: On 1/20/22, between approximately 3:30-4:30pm, when staff #1 was transitioning child #1 from the Infant B to Infant A classroom, they put the infant on the carpet in the infant A room while holding their foot in the doorway. Staff #1 then unstrapped and picked up infant #2 from the swing and took the infant into the Infant B classroom. For approximately 1-2 minutes, when the staff's foot was in the doorway, staff did not maintain sight and sound supervision of the three remaining infants in the infant B classroom.
Plan of Correction: Staff have been retrained in sight and sound supervision at all times.
