During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 5/12/25. The on-site inspection began at 10:25am and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were 71 children present and 14 staff. The inspector reviewed six children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/21/25. A POC submitted after this date will not appear on the public website.
Violations
9The annual fire inspection was more than 5 months past due.
The health inspection was more than two months past due.
On 5/12/25 in the Preschool 2A classroom, the Toddler B classroom, and the Toddler A classroom, the posted allergy list was dated 2/3/25 and contained documentation that there were no allergies in the classroom. The current allergy list for the center was dated 5/4/25 and included dietary restrictions and sensitivities for children in the Toddler B and Toddler A classrooms.
One prescription medication was observed on-site for Child G and one nonprescription medication was observed on-site for Child F. There were no written authorizations from the parent for either medication.
On 5/12/25 a medication was observed on-site without a label indicating for which child the medication belonged. Staff indicated the medication was for Child F.
1. On 5/12/25, two medications were observed in an upper unlocked cabinet in the Preschool 1-B classroom for children two- to three-years-old. 2. Three other medications were observed in unlocked desk drawers in the center lobby.
An annual lockdown drill was more than two months past due.
On 5/12/25 the record for Child G was requested for review. There was no record available on-site for Child G.
1. At approximately 11:05am on 5/12/25, there were 14 children observed in the Preschool 2B classroom for children three- to four-years-old. The attendance record contained documentation of 13 children present. 2. At approximately 11:20am on 5/12/25, there were eight children observed in the 2-A classroom for children two-years-old. The attendance record contained documentation of six children present. A staff member updated the attendance to include the two missing children. The same staff then brought another child to the classroom and did not add the child to the attendance record. 3. At approximately 11:38am on 5/12/25, two children were observed in the Toddler-B classroom for children 12- to 18-months old. The attendance record contained documentation of three children present. 4. At approximately 11:20am on 5/12/25, the attendance record for the Preschool 1-B classroom for children two- to three-years-old contained documentation of children present. There were no children in the classroom. The children had been divided between several classrooms and were not transferred to the attendance in those classrooms.
