Standard 8VAC20-780-130-E
The center is required to obtain documentation of additional immunizations once every six months for children under the age of two years.

1. The most recent immunizations on record for Child B, 17-months-old, were dated when the child was eight weeks old. 2. The most recent immunizations on record for Child D, 22-months-old, were dated when the child was eight weeks old.
Plan of Correction: Family was asked to provide updated immunizations every six months for children under two years old. Moving forward, administrative team will ask for updated immunizations.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, are required to be maintained in a clean, safe, and operable condition.

In the PreK B classroom a plastic cover on the sensory table was observed to have one entire corner broken off, posing a scratching hazard.
Plan of Correction: The lid on top of the sensory bin was moved immediately. Moving forward, management continue to do walk through of the school to ensure there are no safety hazards, in addition staff are required to inform management team if there are any broken items that pose a safety hazard.

Disposable diapers are required to be disposed of in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface during disposal.

The trash can in the bathroom of the Two A classroom did not have a lid and was observed to contain dirty diapers.
Plan of Correction: Teacher removed the dirty diapers out of the trash can. Teacher was informed to dispose diapers only in the trash can that has a lid.
Standard 8VAC20-780-80-A
(Repeat Violation) The center is required to maintain a written record of daily attendance for each group of children that documents the arrival and departure of each child in care as it occurs.

1. On 12/4/25 at approximately 10:35am, four children were observed in the Infant A classroom. The attendance record contained documentation of three children present. 2. On 12/4/25 at approximately 10:52am, four children were observed in the Toddler A classroom. The attendance record contained documentation of three children present.
Plan of Correction: Child was immediately signed in. On 12/10/25, we reviewed keeping attendance records accurate and signed in students upon arrival.
