On 12/17/25 preschool-aged children were observed playing on an indoor climber in the gym that measured 47 inches tall at the highest point. There was no resilient surfacing under or around the climber.

Plan of Correction: All teachers were informed not to use the climber without resilient surface under or around the climber. Moving forward, management continue to do walk through of the school to ensure there are no safety hazards.
