Inspector Notes
An unannounced Monitoring Inspection was conducted today. A total of 8 children (20pts) with 2 staff providing supervision was onsite. One additional child in care placed the provider over the licensed capacity of 7 children at any one time. The children were transitioning to lunch time. They were observed sitting at either small tables or high chairs. No medication needs and no food allergies are identified at this time. Discussed ways in which the transition to food preparation to serving and supervising can be improved by prepping meals ahead of time or relocating the food preparation area to the lower level. A total of 6 children's files, the Provider and the Assistant's file and all household member files( 3) were reviewed. Some violations were found today. Please address a corrective action for each violation and a date to be corrected. This inspection occurred between 11:45am and 1pm. The family day home's current license has licensed capacity of 7 children. During the inspection a total of 8 children were presently in care.

Child #5 and 6' record did not include a start date.
Child #5's record did not include phone numbers for two emergency contacts.
Child #5's record did not include work contact information for one parent.
Evidence:
Child #3 has a topical ointment onsite. A parent authorization was not obtained for this ointment.
