Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interview, and a virtual tour of the program.

A subsidy health and safety inspection (SHSI) was initiated and concluded on February 10, 2022. There were no children present with the Vendor but she has 4 children enrolled. The Vendor is current in CPR, annual training, TB Screening, attendance recording, and emergency drills. A code compliance inspection was also conducted on this date.

Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the program. Please call with questions 703-479-4679

Based on record review, it was determined the Vendor could not locate the immunization record for a child.
Evidence:
1. Vendor has files maintained for each child enrolled but was unable to locate the report for one child.
