A subsidy health and safety inspection (SHSI) was initiated and completed on 10/28/2025. The inspection started at 11a.m. and concluded at 12p.m. There were 3 children present, 2 belonging to the provider and 1 caregiver; the point total was 2. The inspector reviewed 4 children?s records and 1 caregiver/ 2 household members records on-site.
