Inspector Notes
A subsidy health and safety inspection (SHSI) completed on 10/22/2025. The inspection started at 11:20 am and concluded at 11:50 am. There were (2) children present and (1) caregiver; the point total was (5). The inspector reviewed (3) children?s records and (3) caregiver/household members records on-site.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

1The vendor has not maintained attendance as it occurs, there were 2 children in care with no documentation of arrival time. The last documented attendance was 21 days ago.
