Inspector Notes

An unannounced subsidy health and safety inspection was conducted on 6/21/2024 beginning at 10:55am and ending at 11:35am. There were 4 children in attendance at the time of the inspection. Children were observed during outdoor play, snack time, and completing activity sheets. Required background searches, training certificates and household member information was reviewed. A sample of requirements for daily attendance, child information/records, emergency preparedness planning and supplies were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the vendor. Based on review of children's information, the vendor did not obtain the telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached.

Evidence: There was no telephone number listed for the emergency contact of Child #4.
