Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on 6/17/2025. The inspection started at 10:55am and concluded at 11:45am. There were three children present and one caregivers; the point total was six. The inspector reviewed four children?s records and five caregiver/household members records on site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
