Inspector Notes

An annual subsidy inspection was conducted on October 13, 2023 from 2:00p-2:30p. There were no children in care and no children living in the home under the age of 12 with the vendor. There are 4 children enrolled in afterschool care . The inspector reviewed 2 children?s records and 2 adult/household member records submitted by the vendor to ensure compliance with the health and safety regulations.

-Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Based on record review and interview, the vendor did not maintain a written hard copy record of daily attendance.

Evidence: The vendor stated they ran out of attendance sheets and the last recorded date of attendance was completed in 02/2023.

Based on record review, caregivers did not undergo a tb screening every two years from the date of the most recent screening.

Evidence: Caregiver 1's tb screening was last dated 10/5/2021.

Based on record review, evacuation procedures were not practiced at least monthly.

Evidence: Evacuation procedures were not practice at least monthly since May 2023.
