Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on June 3, 2025. The inspection started at 11:10am and concluded at 12:10pm. There was one child present and one caregiver; the point total was 2. The inspector reviewed one child's record and one caregiver's record on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Children's information shall include written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.

Child #1 did not have written authorization for emergency medical care from the parents in their record. The child was in attendance at the time of the inspection and has been in care for two and a half years.
Plan of Correction: The vendor will obtain written authorization for emergency medical care from the parent and place the document in the child's record.

Children's information shall include permission for field trips.

Child #1 did not have documentation of permission for fieldtrips in their record. The vendor takes the child to the neighborhood park.
Plan of Correction: The vendor will use a model form for permission for field trips to the neighborhood park and have the parent to sign the form.
