Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 03/13/2025. The inspection started at 1:20p.m. and concluded at 2:30p.m. There were no children present and 1 caregiver; the point total was 0. The inspector reviewed no children?s records and 1 caregiver 2 household members records on-site

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-260-I
Wood burning stoves and fireplaces and associated chimneys, if used, shall be inspected annually by a knowledgeable inspector. Documentation of the inspection and cleaning shall be maintained by the vendor.

The fireplace within the home is wood burning. An inspection and cleaning has not been conducted in the past 12 months.
Plan of Correction: A fireplace company will contacted to complete an inspection. We may consider closing this permanently.
Standard 8VAC20-790-410-B-2
The following nonmedical emergency supplies shall be required: one working, battery-operated radio.

A battery operated radio was not maintained in the home.
Plan of Correction: We will buy a battery operated radio.
Standard 8VAC20-790-420-A
The emergency preparedness plan shall contain all required procedural components.

The provider's preparedness plan was missing the following components:

A procedure to reunite children with a parent or authorized person designated by the parent to pick up the child; continuity of operations procedures to ensure that essential functions are
maintained during an emergency; and notification of local authorities (fire and rescue, law enforcement,emergency medical services, poison control, health department, etc.), parents,
and local media.
Plan of Correction: The plan will be updated and the model emergency plan will be completed.

A 911 or local dial number for police, fire, and emergency medical services and the
number of the regional poison control center shall be posted in a visible and
conspicuous place.

Emergency numbers were not posted in a visible and conspicuous location within the home.
Plan of Correction: Along with the emergency plan, the phone numbers for poison control and local police will be posted.
