During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on 03/09/2026. The inspection started at 3pm and concluded at 4pm. There were no children present and 1 caregiver. The inspector reviewed 1 caregiver/2 household members records on-site.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
3The provider did not complete the annual department's health and safety update course.
Wood burning stoves and fireplaces and associated chimneys shall be inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Documentation of the inspection and cleaning shall be maintained by the vendor.
The provider did not have documentation that an annual inspection and cleaning was completed on the wood burning fireplace located on the lower level of the home.
The following nonmedical emergency supplies shall be required: one working, battery-operated radio.
A battery operated radio was not onsite.
