Inspector Notes

An unannounced code compliance inspection was initiated on 1/24/2023 and concluded on 1/24/2023 from 10:55 AM to 12:45 PM. There were 68 children present, ranging in ages from 7 months to 5, with 14 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and procedure. A total of three children?s records and four staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

REPEAT VIOLATIONS
Based on record review and interview, the center failed to implement a procedure to ensure all children in the center are in compliance with immunization provisions for two out of three children's records reviewed.

Evidence:

1. The record for child 1 documents the start date as 2/10/22. There was no immunizations in the record.
2. The record for child 3 documents the start date as 11/19/22. There was no immunizations in the record.
3. The center's policy is that immunizations are due prior to the first date of attendance per the parent handbook.
Plan of Correction: The parents of both children were contacted during the inspection to have the immunization records faxed to the center. In the future children will not be allowed to attend until immunization records are received.

Based on record review and interview, the center failed to request a search of central registry background check within the first 30 days of employment and a search of sex offender registry prior to the first day of employment.

Evidence:

1. The record for staff 1 was reviewed. The record document the employment date as 11/16/22. Per the resume for staff 1 in the file up until the hire 2022 staff 1 worked in NJ. The record for staff 1 did not contain a central registry background check or sex offender search for NJ.
2. The director verified they did not complete an a central registry background check or sex offender search for NJ.
Plan of Correction: The background check paperwork will be completed and mailed by 2/1/23. In the future all new staff will be asked if they have lived outside of VA within the last five years upon hire and the appropriate background checks will be completed at that time.
