Inspector Notes

A subsidy health and safety inspection ( SHSI) was conducted on 9/17/2025 from 10:11 am- 11:08 am.
There were 2 children present with 1 caregiver. The inspector reviewed 4 child records and household member records on-site.

The information gathered during the inspection determined non-compliance wit applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-180-B
The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

The provider did not have documentation of taking daily attendance since 7/21/2025 ( approximately 2 months)
Plan of Correction: I cannot find. They were hanging there on board. Corrected during inspection.
Standard 8VAC20-790-200-3
The caregiver did not have a current Tuberculosis screen.

The tuberculosis screen on record had been expired 7 months.
Plan of Correction: I will do tomorrow.
Standard 8VAC20-790-230-A
Before a child may attend the family day home, the vendor shall obtain documentation that the child has been immunized according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations.

Child A who had been in attendance for approximately 2 months did not have documentation of immunization by the first day of attendance.
Plan of Correction: Mommy said she gave me. I called her during the inspection to send.

Areas and equipment of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition.

There was mold on the window sill in the child play area.
Plan of Correction: There is a water leak. I will call to get fixed.

Cribs shall be provided for children from birth through 12 months of age and for children 12 months of age.

A child who is 4 months old did not have a crib to sleep in while in care.
Plan of Correction: I will buy. There is a bed in a box.

The diapering surface shall be nonabsorbent and cleaned and sanitized after each use.

The diaper changing pad had a tear.
