A subsidy health and safety (SHSI) inspection was conducted today between 10:00 and 10:45am.. There were no children present with 1 caregiver/vendor. The physical plant, policies and procedures, emergency supplies and procedures, 1 provider/vendor record and 3 household member records were reviewed. The exit interview was conducted with the provider/vendor. Areas of non-compliance are identified in this report.
