Inspector Notes

An unannounced subsidy health and safety inspection was conducted on March 4, 2024, beginning at 9:50am and ending at 11:30am. There were no children in attendance at the time of the inspection. The vendor's record, training certificates, daily attendance, children's records, household members records, emergency procedures, and first aid supplies were reviewed. The home and outside play area was inspected. No medication on sight. The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the vendor. Based on record review, the vendor did not obtain documentation for a 5-year repeat check.
Evidence:
1. The record for the vendor did not have documentation of a 5-year criminal history repeat check. The last documentation expired on 9/21/23.

Based on record review, the vendor failed to ensure all required information was
documented in each child's record.
Evidence:
1. The record for Child #1 and Child #2 did not have the name, address and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached.
2. The record for Child #1 and Child #2 did not have the name, address, and telephone number for each custodial parent's place of employment.
3. The record for Child #4 did not have the name, address and telephone number for each custodial parent's place of employment.
Plan of Correction: I will have the parent to complete the information this week.

Based on observation, the mattress did not have a waterproof material cover that can be cleaned and sanitized.
Evidence:
1. The twin mattress used for Child #4 did not have a waterproof material cover.
Plan of Correction: I will buy a waterproof mattress cover this weekend.

Based on observation, the battery-operated flashlight was not working.
Evidence: The flashlight was corroded with rust and would not work when new batteries were installed.
Plan of Correction: I will get my husband to give me a new flashlight.

Based on observation, there was not a working, battery-operated radio.
Evidence: The battery-operated radio was corroded with rust and would not operate after installing new batteries.

Based on record review and interview, the evacuation procedures were not practiced at least monthly.
Evidence:
1. The last documentation of an evacuation drill was dated 5/12/23.
2. The vendor acknowledged the evacuation drills had not been completed.
