Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on 5/1/2025. The inspection started at 12:15pm and concluded at 1:10pm. There were no children present and one caregivers; the point total was zero. The inspector reviewed three children?s records and five caregiver/household members records on site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
