Inspector Notes
An in-person annual subsidy inspection was conducted on 6/17/2024 from 9:50am to 10:35am. There was (1) child in care with the vendor. The inspector reviewed 2 children's records and 3 adult/household member records submitted by the vendor to ensure compliance with the health and safety regulations.
Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Based on the review of the vendor records, the vendor failed to get a subsequent background check after 5 years.
