During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 12/02/2024. The inspection started at 9:24am and concluded at 10:08am. There were three children present and one caregivers; the point total was seven. The inspector reviewed two children?s records and one caregiver/household member records on-site on 12/02/2024.

This inspection included document review, tour of the home and observations:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 12/09/2024.

Standard 8VAC20-790-250-F
Caregivers who work directly with children shall, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training, to include the department's health and safety update course.

The caregiver completed the health and safety update course 1/18/2023 did not complete the department's health and safety update course annually.
Plan of Correction: I finished the training today.
Standard 8VAC20-790-270-A
Potentially poisonous substances, materials, and supplies such as, but not limited to,
cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and
petroleum distillates shall be stored away from food in areas inaccessible to children.

Five cleaning products were under the kitchen sink in a cabinet accessible to children. Two cleaning products were on top of the kitchen counter and accessible to children.
Plan of Correction: Everything was placed Immediately in the right place.

The diapering surface shall be separate from the kitchen, food preparation areas, or surfaces used for children's activities, nonabsorbent and washable; and cleaned and sanitized after each use.

The diapering surface was a cloth material that was absorbent.
Plan of Correction: I ordered and changed the Changing table pad.

Lockdown procedures shall be practiced at least annually.

The vendor did not practice a lockdown procedure in 2023.

The vendor maintained a record of the date of the practice drills for 10 months, but did not document the drills for October 2024 and November 2024.
