Inspector Notes

An in person Subsidy inspection was conducted today. There are 5 children preset at Inspector arrival (1:45PM). Paperwork for children, vendor and residents was reviewed. Children were observed resting in the basement and quietly playing in the dining area of the kitchen.
Please email to me by 6:00 PM tonight: TB documentation, rabies vaccination, 3 hour annual training certificate and immunization record for 1 child.
I will send you forms that need to be completed for the Family Day Home license application.

Based on direct observation the vendor did not ensure that the family day hoe does not exceed the capacity of children cared for by law or regulation.
Evidence:
At Inspector arrival (approximately 1:45PM) there were 5 children in care (13 points).
The permit for Fairfax County sets the capacity at 4 children.
Plan of Correction: I will apply to get a license.
