Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on 1/17/2025. The inspection started at 11:00 AM and concluded at 12:05 PM. There were 3 children present and 1 caregiver; the point total was 6. The inspector reviewed 3 children?s records on-site and 5 caregiver/household member records electronically.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
2Household Member A had both a Central Registry Search and nationwide fingerprint search that were overdue by 4 months.
There was no current rabies vaccination available for review for the dog.
