Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated on 3/11/2026 and completed on 3/11/2026. The inspection started at 9:40am and concluded at 10:30am. There were 2 children present and 1 caregiver; the point total was 3. The inspector reviewed 1 child's record and 1 caregiver/household member(s) records on-site on 3/11/2026.
This inspection included document review, a tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
