Inspector Notes
An unannounced annual inspection was conducted today. There were 4 children present (7 points) with one caregiver.
2 child records were reviewed. See Violation Notice for missing items.
Children were observed playing with toys, building a fort, watching a tv show, washing hands and eating lunch. The menu is sent to the parents digitally.
Inspector was present today from 12:05 PM - 1:10 PM.

Based on a review of records, child records did not contain all required information.

*Child's full name and address.
*Name, address and telephone number for each custodial parent's place of work
*Name address and telephone number of at least one emergency contact.
*Name of persons other than custodial parent who are authorized to pick up the child
*Written authorization for emergency medical care.
*Permission to transport the children
*Permission for fieldtrips
*Written statement that the vendor will notify the parent when the child become ill and have the parent arrange to pick up the child if requested.
