Based on a review of children's records, the vendor did not obtain documentation that all children have been immunized according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations.
Evidence:
1. Child #2 did not have immunizations on file at the time of the inspection.
Plan of Correction: I gave it to the parent because they switched to a new doctor and needed to take it to the visit today, but she will bring it back tomorrow.
Standard 8VAC20-790-410-A-1
Based on observation, the first aid kit did not contain the minimum requirements.
Evidence:
1. The first aid kit was missing the following supplies: band-aids, tweezers, scissors, and thermometer.
Plan of Correction: I will buy the items that are missing.

Based on observation, the family day home did not have the required nonmedical emergency supplies.
Evidence:
1. The battery-operated radio in the family day home was not working at the time of the inspection.
Plan of Correction: I will get more batteries so it will work.
