Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on 8/27/2025. The inspection started at 1:35pm and concluded at 2:20pm. There were 3 children present and 1 caregiver; the point total was 8. The inspector reviewed 3 children?s records and 1 caregiver record on-site on 8/27/2025. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
3Caregiver #1 did not have documentation of a tuberculosis screening on file at the time of the inspection.
There was no documentation of any evacuation procedures practiced for February, March, April, May, June, and July 2025.
The dog living in the home did not have documentation of a rabies vaccination on file at the time of the inspection.
