A subsidy health and safety inspection (SHSI) was conducted on 10/24/2024. The inspection started at 11:25am and concluded at 1:05pm. There was one child, one household member, and the vendor present. The inspector reviewed 4 children?s records and 7 vendor/household member(s) records on-site.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today.
Standard 8VAC20-790-180-B
The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Attendance was not recorded for the current week.
Plan of Correction: They were filled out.
Standard 8VAC20-790-190-B
The vendor shall maintain information for each child which shall include proof of identity and the name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached.
Child #1, enrolled for more than 3 months, did not have documentation that proof of identity was verified. The emergency contact for Child #1 was missing the address and telephone number.
Plan of Correction: The paper is now filled and will be sent.

Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.
Two electrical outlets in the sitting area did not have protective covers.
Plan of Correction: They will be all replaced with non-choking covers.
Standard 8VAC20-790-400-H
Medication, except for those prescriptions designated otherwise by written physician's order, shall be kept in a locked place using a safe locking method that prevents access by children.
Household medications found in the kitchen area were not stored using a safe locking method.
Plan of Correction: The medications are placed away in a locked storage.

The vendor shall maintain documentation of the current rabies vaccination for dogs and cats.
The vaccination record for the household cat was not available at the time of the inspection.
Plan of Correction: The vaccination records were emailed to the inspector.
