During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on 05/27/2025.
The inspection started at 12:00 pm and concluded at 1:00 pm.
There were 2 children present with the Vendor; the point total was 4. The inspector reviewed 2 children?s records and the Vendor's record on-site.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
3The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
At the time of arrival, the Vendor did not have an attendance record for the day. Upon request, the Vendor could not provide the attendance record for previous days. The Vendor stated that she had disposed of the attendance record.
An up to date record for each child enrolled shall be maintained by the Vendor.
Children records were missing the following components:
1) Proof of the child's identity - Missing in the records for Child #1 and Child #2.
2) Information on allergies was missing in the record for Child #1.
Potentially poisonous substances, materials, and supplies shall be stored away in areas inaccessible to children.
On the date of inspection, the following products were observed in areas accessible to children as follows:
* A bottle of hand sanitizer on top of a tv console in the living room area.
* In the hallway restroom - Various bottles of hair shampoo and hair conditioner on the bathtub; a bottle of hand lotion, a bottle of dish soap on shelves by the toilet.
* In the bedroom - men and women deodorant , various perfume bottles, a Cetaphil bottle, vaseline, eyedrops and various cosmetics.
