A Subsidy Health and Safety inspection was conducted on 12/15/2023 from 8:00 am- 8:50 am. There were 4 children directly supervised by the vendor. During the inspection the home, activities, emergency preparedness planning, drills and supplies were inspected. The children's records, vendor information and household member information were also reviewed.
Information gathered determined violations with applicable code or requirements and are listed on the Violation Notice.

Based on record review, vendor did not maintain a written hard copy of daily attendance that documents the arrival and departure of each child in care as it occurs.
