Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interview, and a virtual tour of the program.

A subsidy health and safety inspection (SHSI) was initiated on January 25, 2022 and concluded on January 28, 2022. There were 2 children present with the Vendor present. The Vendor needed to locate clearances and they were submitted for review. A code compliance inspection was also conducted on this date.

Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the program. Please call with questions 703-479-4679

Based on record review it was determined that the Vendor is not have current certification in CPR.
Evidence:
1. CPR card has an expiration date 1/11/2022.
Plan of Correction: I am scheduled for class the first week in February.
