Inspector Notes

An unannounced subsidy inspection was conducted between 10:30am and 12:05pm. There was 1 child present = 4 points with the Vendor at the time of the inspection. Ratios and supervision were in compliance. There was also an adult household member. The daycare was sufficiently supplied with toys and equipment for children and appeared clean and organized today. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Based on review of child records, one child record did not include the required information.
Evidence:
Child #1's record did not contain the provider's sick policy, immunizations and parent permission for field trips to the local park.
Plan of Correction: I will request the shot record from the county.
Standard 8VAC20-790-230-A
Based on child record review, documentation that one child has been adequately immunized was not obtained.
Evidence:
No documentation of immunizations were found for Child #1.
Plan of Correction: I will get the shot record from the county.

Based on observation, hand washing was not observed before feeding one child.
Evidence:
The provider was observed bottle feeding Child #1 and did not wash their hands prior to feeding the child.
Plan of Correction: I will remember to wash hands before feeding any children.
Standard 8VAC20-790-440-F
Based on observation, food was not served in a clean and sanitary manner.
Evidence:
The provider removed the top of the infant bottle and placed the top on the floor in order to pour formula into the bottle. The provider placed the top of the bottle onto the infant bottle without sanitizing the top and proceeded to feed Child #1.
Plan of Correction: I will not do this again.
