8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes
Inspector Notes
An announced reinstatement subsidy health and safety inspection (SHSI) was conducted at 10:00am. Observed 1 child = 2 points with the vendor. Ratios and supervision were in compliance. The child was observed playing with toys, watching a child-appropriate TV show, washing hands, eating a snack, and departing for the day. The daycare areas of the home were well supplied with toys and equipment for children. Areas of non-compliance were found with subsidy standards and laws reviewed. Based on records reviewed, it was determined that the vendor did not ensure that a household member obtained a central registry search within the required timeframe. Evidence: Household member #2 was required to obtain a central registry search within 30-days of 5/10/21.
Plan of Correction: The vendor said she submitted the paperwork about 3 months ago and has not received a response. The vendor was advised to work with the local agency that facilitates this paperwork on her behalf to complete the background check. Remains in process.

Based on records reviewed, it was determined that the vendor did not ensure that documentation of immunizations were updated every 6 months for children under 2 years old.
Evidence:
1. Child #9 - The immunizations on-file were dated January 2020, when the child was 13 months old.
2. Child #10 - The immunizations on-file were dated April 2021, when the child was 6 months old.
