Inspector Notes

An annual Subsidy Health and Safety Inspection was completed on 7/28/2022 from 10:15 am-11:10 am. There were 4 children and the vendor's child directly supervised by the vendor at the time of the inspection. Child information, provider and household member information and trainings were reviewed. Emergency preparedness planning, supplies and drills were also reviewed. Diaper changing routines, handwashing routines, napping cots and age and stage appropriate activities were discussed. Cleaning and sanitizing requirements were reviewed for all areas that children would be using including basement play area, dining area, and family room on the main level.

Information gathered determined Violations with applicable code and/ or requirements and are noted on the VIOLATION NOTICE.

Based upon record review and interview, required written hard copy of daily attendance that documents arrival and departure of each child in care as it occurs was not completed at the time of the inspection.
Evidence:
Child A, Child B, Child C and Child D 's arrival into care was not documented on 7/28/2022.
Plan of Correction: I was out of town. I will start back up again. Corrected during inspection.

Based upon observation, areas of the family day home were not maintained in clean and safe condition.

Evidence: Upon inspector's arrival into the home pieces of trash, water bottle caps, empty hand sanitizer bottles, and coins were found located in the TV room on the main level where children were observed playing.
Plan of Correction: Items removed during inspection. I was out of town I will clean up.
