Inspector Notes

An unannounced annual Unlicensed Subsidy Health and Safety (USHSI) inspection was conducted on 02/02/2033. There were 3 children (10) present with the vendor.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 3 child records, the Vendor's record, and 1 household record were reviewed to ensure compliance with the health and safety regulations.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review and interview with the vendor, the vendor did not annually attend at least 16 hours of training, to include the department's health and safety update course.
Evidence:
1) On the date of inspection, the vendor had completed 7.5 out of the required 16 hours of annual training for the year 2022.
2) The most recent health and safety update course certificate in the record for Staff #2 was dated 01/04/2020.
Plan of Correction: The health and safety update training for 2022 was completed on 02/11/2023.
