Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on 06/30/2025. The inspection started at 1p.m.and concluded at 2:00p.m.. There were no children present. The provider and 1 household member was present during the inspection. The inspector reviewed 1 caregiver and 2 household member's record on-site.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
1Caregivers who work directly with children shall complete the department's health and safety update course.
The provider has not completed the 3 hour health and safety update course for the past two years.
