Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated on 8/18/2025 and completed on 8/18/2025. The inspection started at 10:30 am and concluded at 11:41 am. There were no enrolled children present; 4 family members, and 1 caregiver were present; the point total was 1. The inspector reviewed 2 children?s records and 2 caregiver/household member(s) records on-site.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/22/2025.

The record for the vendor did not contain documentation of the annual 3-hour updated subsidy training course for 2024.
The vendor did not record the date of the lockdown practice drill for 2024.
