During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted on 1/17/25. The inspection started at 11:50am and concluded at 12:35pm. There were 2 children present and 1 caregiver; the point total was 4. The inspector reviewed 2 children?s records and 2 caregiver/household member(s) records while on-site. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

There was no documentation that the proof of identity had been verified for Child #8 and #9.
