Inspector Notes
A Subsidy Health and Safety Inspection was conducted on 7/14/2022 from 10:54am-12:25pm. There were 4 children directly supervised by the provider. During the inspection the home, interactions with children, emergency supplies, procedures and drills were
inspected. Cleaning and sanitation routines, hand washing and diaper changing routines were also discussed. A sample of child records, provider information and household member information was reviewed. 1. Child #1 was missing documentation of the authorization for emergency medical care and verification of proof of identity.
2. Child #2 was missing documentation of the authorization for emergency medical care and an address for one required emergency contact listed.
3. Child #3 was missing documentation of the authorization for emergency medical care and an address for the emergency contact listed.
4. Child #4 was missing the authorization for emergency medical care and the name, address, and phone number for one required emergency contact.
Evidence:
1. bin containing medications was located on top of the refrigerator.
2. A bag containing medications was located in an unlocked closet in the daycare room.
Evidence:
1. The first aid kit was missing tweezers, gauze, and first aid tape during the time of the inspection.
