Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on July 2, 2025. The inspection started at 10:00am and concluded at 11:00am. There was one child present and one caregiver; the point total was 4. The inspector reviewed one child's record and three caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Repeat Violation

Vendors shall maintain written or electronic information for each enrolled child.

Child #1's record did not include the name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached and written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. Child #1(9 months) was present at the time of the inspection and has been enrolled in care for one month.
Plan of Correction: The vendor will obtain the name, address, and telephone number of an emergency contact from the parent. Written authorization for emergency medical care will also be obtained today.

Caregivers who work directly with children shall annually complete the department's health and safety update course.

Caregiver #1 did not complete the department's health and safety update course for 2025. The department's health and safety update course was completed 14 months ago.
Plan of Correction: The vendor will complete the department's health and safety update course by July 15th and send certificate to the licensing inspector.
